Very common for a .22 semi-auto to do this. Part of the .25 ACP's development was to answer this problem.

Not uncommon. 22 actions tend to foul really bad. Try giving the action a good cleaning. If the feed ramp/chamber throat area gets gummy or fouled it can cause this. I have an old Marlin that was doing the same thing. Took it apart and put a bore brush in the chamber with a good solvent, problem went away.
