Also, what if a person weighs 210-215 and runs 2 miles at 16:00 or less. How are they fat or out of shape?

Weight really only matters for body fat percentage. A guy could have 18% body fat and still be in good shape.
There are different types of fitness, tho. Cardiovascular, flexibility, core, and strength. I have a buddy who runs the Boston Marathon and he probably can't do one pullup. I can do pullups all day but would be eating his dust in a 5k.
I guess it all depends on what your goals are.

It depends on the person and his build/fitness/goals/etc.
The average American male who is 5'11" and 205+ is, at the very least, chubby and getting towards the fat end of things. That's not to say that these monster NFL players are out of shape or anything, it's just that 98 out of 100 Americans at that height are/look chubby or fat at that weight.
I am 5'11". I had surgery last year and ballooned up to 185 - 190 when it was all said and done. I looked outright chubby at that weight. I'm back down in the 165 - 170 range and look/feel/am a hell of a lot healthier, stronger, fitter, have more endurance, and so forth. I'm probably just a hair on the slim slide by natural build, but not by much. Your average dude carrying that weight at that height is not in good shape.
