My wife has chronic migraines.
She takes Nortriptyline on a daily basis. Also, when migraines become unbearable she has a prescription "cocktail" consisting of Hydrocodone, Zofran and Benadryl. Pounds the water during the flare ups as well.

for me, imitrex, Benadryl, Tylenol, and motrin with a coca cola in a can ice cold followed by a nap works wonders.
as far as your wife goes, additional things yall might try: verapamil and nortryptilin (as has been suggested) and also Neurontin
