When is the test? If it's the next class or a few classes later, just feign ignorance that you washed the email address accidentially the next time you see her.
Follow it up with.. If I had your number, I would have called you. Since you wouldn't have left your phone in your pocket to get laundered.

I would love to hear the chicks version of this story. I am guessing she would tell us how some creepy dude watches her out of the corner of his eye during class. That he got all pissy when she refused giving her personal info after he tried to get her email with an offer to give her his notes. IMO OP likes her and is pissed she isn't returning the interest.
