Is your gas always on? Are you a "seasonal" customer? If so, your line usually has air in it the first time you try to light it for the winter...if so, just keep at it until the gas takes hold. Otherwise, you may have a fireplace issue.

who is they? Atmos? I doubt you have a separate line going to your fireplace that they would shut off. Go in the attic, or wherever the gas line comes in, and follow it to the different locations.

Nope...you have to call and request this! The only problem is they vary the turn on charges each year...30 bucks last year and they went back up to 60 this year...I still save a little over the maintenance fee however.
Question, do you smell gas when you open the valve by your fireplace while trying to light it? Are you using an igniter or a match to light it?
