I think the layout is good, but the verbiage sounds a little too much like a bill rather than a donation to me...

I would replace every instance of "payment" with "donation"

I would also give a cheaper option where you would put the company logo on literature to be handed out to the team (maybe schedules, etc.) or something like that....
