I'm assuming this is something they deal with on a daily basis. I would think that they are more informed to prevent problems like this. If they do not know, then they should have contacted the OP and let him find out himself. I guess I'm giving them too much credit though

Do you know how many insurance companies there are? And different plans and plan changes?

A drs office can not be expected to know which lab is or is not in network. Maybe there should've been more communication but I don't see how the doctor would've known

Yes, get a certified copy of the policy to begin with and read it. then base the appeal on the policy language
