Another consideration is the size and scale of the federal government's health system.
DoD bases, NIH, VA, et al., these all carry lower salaries than the private sector, dramatically so.
There are a lot of civilian providers working in this capacity. Much of their compensation is in the form of benefits, not just wages. Retirement benefits being the most prominent.
