in my seven years with my current company, i've never worked on a hard-bid project... just negotiated (either lump sum or cost plus)... in each case, the Owner has hired the testing agency.
It's usually done before we ever hit the job... whoever does the geotech report (which is generally included in the specs) is usually who ends up doing all the testing -- soil density, concrete, rebar, etc.

Just an FYI. This is a job I bid last week. And all of our work is either negotiated or invited bidders only.
