On a throw in I guess a foul is called if a guy doesn't throw it exactly in the middle of the two teams hoisting up their one man? Does the throwing team still try to get the ball more towards their guy?

What determines how and when a ball is thrown into a scrum?

Yes they are supposed to zip it down the middle

Also when the ref says 'yes 9'
That's when the fly half (#9) rolls the ball in the middle of the scrum

But just like the line out, guys have learned how to put a little English on the ball

Correct, it has to be down center. It's usually up to the refs discretion.

Usually when there is a knock-on(when the offensive player fumbles the ball forward). And when the ball goes dead in the endzone their will be a 5 meter scrum.
