Vacuum hose has a hole in it or has come off. I had an explorer with the same issue

I have the exact thing with my 04 F150. I was told it was my "Cam Phaser"? I do know my timing tensioners need to be replaced as I hear some slapping of the chain when it first starts up.
My truck would run fine until it heated up and then like the OP , when low idle it would shake and die if I did not put it in neutral and give it some gas.
I would really like for it to be a cheaper fix. I was told that the "cam Phaser" (whatever that is ) is like a 2500 job. ?
