If referring to throwing back to a base after a pitch or at the end of a play yea. But it also can mean throwing to base even though you don't have a shot to get the runner.

Doesn't matter either way, what matters is it was a terrible decision, and then equally terrible to not catch the throw

This is the only way I've ever heard throwing behind a runner referenced.

You're right, a perfect throw doesn't get him, but that happens a lot in baseball. It was close enough to warrant a throw, especially when the guy running has a bum ankle.
