It just means that the team has to wait for the refs to move the ball, start the clock, etc. I've seen several times where Brees and Peyton have sat there at the LOS waiting for the refs to signal they are ready.
This is why I don't think Chip Kelly's offense is going to be that revolutionary for the NFL like some are believing other then being up tempo with more read option looks. There are plenty of teams that run a high tempo offense already in the NFL and you can only go so fast.
