He doesn't talk to anyone else the first season. They thought they would get cancelled and they would reveal it on the last episode. Re-watching the first season. Mind blown.

I remember the commentary talking about that. It was about halfway through season 2 I believe before anyone else interacts with him.

Not really. Bill Lawerence said that if they didn't get renewed that he would've made Janitor a figment of JD's imagination.
But they got renewed so he didn't have to.
