Appreciate the props my man...Once you get the head and face in the right position, you make it semi-transparent and then erase the face where the fingers are, as well as the part of the neck where it meets the collar. bring it back to full opacity, and then it helps (in a picture like this) to slightly blur the face so that it looks like it's further away (giving it depth of field).
