If I were you, I would go to the IRS website and look into it. Instead of about.com.
ETA: read the instructions that come with the W9 form...it talks about what to do.

I just asked my CPA and he said he fills them out both ways. He told me to just keep my company name on the NAME line and choose LLC and then enter my EIN. He just told me to make sure I stay consistent on all my W9's.
