I actually read the book and found it much more enjoyable. I'm one of the movie board posters who don't find the movie very good. I watched it once but I didn't get the hype either.
From the book, I don't believe the Father was a boyfriend at a younger age. He's basically a pedophile if I recall correctly. She lets him hug her and rub her (not too graphically, just sort of touching) if he drains for her. I honestly don't remember why she makes him do it, but I do think there was a reason.
Also, in the book he lasted a while because he'd usually try to get people alone. He only did a few in the book, but I believe it was mentioned he had done more before that.
As far as not living in a large city, I'm not sure why. I think they sort of move around like drifters.
Overall, I enjoyed the book but found the movie sort of boring.

In the book, after it was revealed that she was a he, did they still refer to him as a girl? Would you even consider her a girl now?
