I've been beta testing games for years upon years. For example, I beta tested The Sims Online years ago, and it was fixed and tweaked until the release, but when it finally did release, there were bugs none of us had ever encountered like all of your Simoleons just...disappearing. You can't test every bug out because you'll get bugs that you can't repro or there'll be isolated incidents.
Look at Halo 2's superbounce. That was undetectable until someone accidentally did it. It was unfixable also. Software is incredibly hard to fully test too.
