So a little digging yielded a page from the Los Angeles Department of Mental Health (the jokes write themselves) from 2022.
It appears that the purple circle is for mental patients who identify as Intersex.
I would posit that the Intersex category is likely "ignored" because no reasonable adult could define this specific mental illness.
