Using chainsaws while on a ladder is way up there.

Some things that haven't been mentioned:

bench grinder without protective shroud
oxygen/acetylene torch
band saw
welder
old power tools (before double insulation) with sketchy wiring

I've never really had any issues with a band saw. The only sketchy thing I've seen happen with one was in the first machining class I took. We had to cut rough blanks from something like 4"×3/8" flat stock and some dude was just leaning on the cut, putting all his weight into it. The saw sounded terrible, and it was clear it needed a blade change. I guess he didn't know because he just kept leaning on it. It finally bound, jerked the piece out of his hands, and broke the blade. He had a nick on his finger with a little blood, but nothing major. I'd blame that on the instructor, though.

He learned how to change a blade and what a blade spot welder was that day.
