Like you said, modifying the body to pretend the opposite of reality isn’t going to lead to peace and internal tranquility. We don’t agree with anorexics that they’re morbidly obese and encourage them to lose even more weight.

They are grasping at straws because we've been conditioned to believe doctors and medicine can fix anything. Most of the time, we're right. But not all the time.
Like I said, it's just sad. It's a horrible existence, so I don't blame them for trying.
