100% social. He is either getting bullied or there is a girl he likes. Take him to some MMA classes and he will work up an appetite. Plus he will be able to solve any bullying problem on his own and he will get in shape which will help with his girl situation.
Here is another thought. Maybe he discovered porn and fapping and is too busy to eat.
