No they wouldn’t have to pay taxes in both countries. Just their country of residence. If they don’t work or live in the US, there’s no requirement. No for healthcare benefits. They wouldn’t be able to use the healthcare benefits because they don’t live here.
Your biggest thing to look at is compliance in the prospects’ country. This is a situation where the technology and practice is way ahead of the laws and regulations. there’s a lot of gray area and little enforcement.
