Do the surgery. I kept trying to get by and it never improved. It's been a few years since I did mine so things may have changed, but you can expect to have your arm strapped to your body in a sling for like 4-6 weeks and then you'll have limited range of motion for at least a couple more months.
I think I was back to full strength (and maybe even better) within 3 months, but I was doing PT a few times a week and going to the gym. If you just do PT once a week and nothing else, it's going to take a lot longer. Cary Winder did mine and I'd recommend him to anyone.

Dr winder did my surgery as well. Highly recommend.
