I think he's talking about either the homosexual messaging throughout the game or the over-sexualization of it. I think the term "incel" is silly, but the amount of sex and sex-related things in this game makes me think the vast majority of coders making this are indeed incels.

The game is fun, but damn... I actually have to work to not get my butthole raped by Gale or Halsin as they seem to take any positive response as potential to rub wieners (I don't even give Astarion a chance because he's already projecting).

And then the work to include the ability to swap genitals and choose from genital hair density while not having any ability to vary height or weight and locking color choices... the choices seem rather odd unless you allow for messaging.
