I thought about this and initially agreed with you when I first saw it. Then I remembered Worf got nothing but a demerit for hunting and murdering Duras in front of Riker. It was out of revenge but Worf was the aggressor and got little more than a scalding for killing the leader of one the most important families in the Klingon Empire.

I do agree with the point that turning the doctor into some super soldier was a weird character turn to take with him. I think they implied it was his life before medicine, but still a weird decision.
