Top 3 for me are decomposing bodies, freezer full of fish after 10 days with no power, and vaginal trichomoniasis.

The encounter with trich happened when I was doing a round of clinicals for my paramedic course and some lady came into the ER with severe abdominal pain and turns out she had a severe case of untreated trichomoniasis. They got her legs up in some stirrups and I got a glimpse of the green discharge mere seconds before the smell hit me. The ER doc on that evening was even gagging. He later told me that was one of the grossest things he had ever encountered in the ER and he had been doing it for nearly 15 years.

I'll never forget that smell for the rest of my life.
