Not how it works. Were you around when Nixon resigned and Gerald Ford chose Nelson Rockerfeller to be the vice president? John McCormack didn't become veep when JFK was killed and LBJ became president. The SOTH doesn't automatically become vice president if the latter ascends to the presidency.

It’s relatively high possibility I think in this particular scenario
Biden has to know that he is mentally challenged. His family cannot be happy about the prospects of being embarrassed daily if he gets elected. They must be discouraging him from accepting the nomination

Or better yet, were you around when Vice President Spiro Agnew resigned? The Vice President position is then filled constitutionally using the 25th amendment. The President nominates a replacement and the Senate and House confirm.
Gerald Ford is the only person to become President without having been voted into either the presidential or vice presidential office by the Electoral College.
