What happens then? I can see this happening with the massive mail in ballots, harvested ballots, and dead folks voting.

It has happened before in select counties but not on a wide basis. As far as I can remember nothing happened

They didn’t do anything in Orange County in 2018 when this happened. They aren’t going to do anything this time either.
