HR sent out a vaccine status survey to everyone. Not anonymous

At first I thought they were just collecting general information. But it requires you to put your first and last name.
Options are
Fully vaccinated
1 dose with plans to get 2nd
1 does no plans to get 2nd
Not vaccinated but plan to in near future
Not vaccinated and no plans to
They have not said if the survey itself is required, yet.

This happened to us about 2 weeks ago, then I had to upload my card to the records portal.
The end game of all this scares the frick out of me.

When you send back your answer just say " I'm Black/ Mexican/gay, frick off".

HIPPA is long gone, it's also a violation to take daily temperatures of people and ask them if they have symptoms or have been around anyone with COVID. Yet we have been doing that everywhere in this country for going on 2 years.

I'm an idiot for what again? Not uploading a form I already have to my work's portal that's for that purpose?
I get the sentiment about saying it scares me then just complying, but both things are not mutually exclusive.

I would tell everyone to pick not vaccines and no plans too. It would help keep from them forcing everyone to vaccinate. You need them scared as possible.
