This typically happens when the government heavily regulates an industry. The only players left in the field are large corporations.
Govt forces costs that small and medium players cant cover. In this case, it probably has more to do with the insurance scams - which got a lot worse since 2010.

To say that decisions are in individuals hands without acknowledging that insurance companies tie your hands behind your back is disingenuous at best. Try going to a doctor that's not in your plan, or try getting prescribed a drug that's not covered in your plan. How about a hospital stay that lasts 4 or 5 days? Basically, serious illness equals financial disaster for about 90% of Americans.

Exactly. The only outlier that I know is a pediatric neurosurgeon that got a helluva an offer after his fellowship. That being said, I believe only 7 of these fellows come out to practice annually.

An insurance company not covering something does not prevent you from receiving it. It just prevents them from paying for it.

You’re confusing coverage with access when they are not the same. You’ve chosen and are paying for a plan that covers one thing, you want something else but you want someone else to pay for it.

Neurosurgeons CAN make 800k out of training (Can be up to 16 years of training)and make more than pediatric neurosurgeons. The trend to leave private practice started to gain steam in the late 90's and early 2000's. Also, the life of a busy neurosurgeon is all work, no way I'd want that.
