Yes - get a large jersey if you wear a medium T-shirt. I think one size up is generally the rule for basketball jerseys

I wouldn't... There is more reliable sights for a few extra dollars. Most their jerseys have been cheap jersey material that does not stretch at all. Although my long sleeve Penguins jersey turned out great.
