Literally the first thing I thought about (aside from his insane racist rant) when I saw Richards mentioned.
Go watch any number of Seinfeld blooper reels on YouTube and he comes off as a bit of a dick. Jerry, Jason Alexander, and Julia Louis-Dreyfus are usually cracking each other up - and purposely trying to crack up JLD - and Richards is always like "come on, guys" like he's some method actor.
On a side note, when Jerry Stiller died, they talked about how he was always trying to make Dreyfus crack, especially in the episode where Elaine and Frank Costanza get into a fight after George gets arrested for bootlegging movies. It took them like 15 takes because Stiller kept saying his line a different way, and JLD lost it every time and couldn't keep her composure.

Sarah Holcomb is a weird, sad story. She was legitimately in and out of psychiatric hospitals after Caddyshack, and became a recluse.
Shelley Duvall similarly fell of the face of the earth until she did a bizarre interview with Dr Phil a few years ago. She was clearly mentally ill and barely recognizable. I think she was still acting into her 50s, so it's kind of hard to fathom what happened to her.

My sister used to live near The Comedy Store, and she said people there would tell stories about Richards showing up unannounced to different clubs, and expecting stage time just because he was Kramer.
They claimed he didn't even really have new material to work out; he would just wing it. If he bombed, he'd lash out at the audience. This was before the Laugh Factory incident too.
That said, Jerry is still apparently friends with him, so maybe reality knocked him down a few pegs and he isn't such a tool anymore.

Too much, too soon and she didn't have the tools or infrastructure to deal with it (schizophrenia and a shite ton of drugs do not mix - see Syd Barrett, Jim Gordon, etc.)

Both Amber Heard and Johnny Depp are fairly removed from reality by most accounts. Depp seems to think he is something like his friends K Richards or HS Thompson and Heard took a crap in his bed just to frick with him, threw stuff at him (causing injury and film delays but still wanted to play victim)
Gary Busey and Steven Segal are fairly bonkers. Busey from actual brain trauma I guess. Segal believes his own bullshite and was accused of locking up a woman as a sex slave.
Shirley McClain had some specific reincarnation stories that she got very mad at Letterman for asking about.
Will Smith and Tom Cruise have said some crazy stuff which might be Scientology byproducts.
Charlie Sheen had his own special level of crazy around the time he was fired from 2.5 men.
Bill Cosby had some pretty weird stories circling before he finally went down for being a serial rapist.
