Try to not move your head back when shifting your weight.
Try to think that you are swinging inside a garbage can and you want to stay within that area. Not shifting from back to front

It's a baseball swing through and through, that downwards shoulder jerk when you transition towards your downswing gets you somewhat on plane. How low is yor ball flight with your irons?
