Not on tv, but heard one on radio about 15 years ago or so. I was listening to a Panthers game, think they were playing the Falcons. Panthers had the ball, and it was between plays. I guess the microphone picked up one of the players or coaches that yelled "we gone run this motherfricker" really loud a couple times over the air.

Mark Recchi when he was holding the Stanley Cup after winning it with the Bruins gave a pretty loud "frick YEAH" that got by the censors

Saints/Rams playoff game in 2000. Hot mic caught someone saying motherfricker on the field. It was loud as hell on the broadcast and even Dennis Miller chimed in that it wasn’t him
