Simple IRA with old job

I have a simple IRA I opened with a former employer. It's been over two years, so if I withdraw, I won't get the 25% deduction penalty. It's right around $860, so if I do withdraw, how much should I take out for state(Louisiana) taxes? I make around 70K a year.
What are y'all thoughts of rolling it over to a personal IRA? Any IRA suggestions?
