Pancreatitis.
When a grown man with a high pain tolerance is curled into the fetal position in a corner of the room, propelled there from writhing, sweating and eyes pouring forth tears unable to speak and barely able to wave his hand, that is pain!
I've driven a 5 speed and walked up and down stairs for 6 hours on a right foot broken in 2 places, I've unrolled a paperclip and heated it over a flame to burn off a persistent wart, dislocated joints, dental surgeries...nothing comes close to pancreatitis in my experience.
