There is no way he knew it was poison that fast IMO. And he was behind the barrier, he came from the back. He wasn't watching what was taking place.

Sansa was indeed the only one onscreen with her hand over the cup, right? I'm pretty sure this poison is the same that killed maester cressen and when he poisoned his wine, he let it sit there for a moment before trying to trick Melisandre into drinking it with him. Maybe there is a fermentation period for the poison to activate, thus explaining the time between her hand over the cup and Joffrey beginning to choke.
Old lady Tyrell fondles Sansa's necklace then the next scene is showing a missing gem from the necklace:
Maybe the old lady was fondling her necklace because she noticed the missing gem instead of her taking it. Sansa would be the culprit and she obviously has a world of motive. More than any character. Joffrey made her life miserable, ordered Ned's execution, had Lady killed, and by extension Robb killed as well.
If she did do it, she somehow found out about the poison in the necklace and just took the opportunity when she was able to pick up Joffrey's cup (she didn't plan how everything played out).
This would all include the fool helping as well. He rescued her at the end because maybe she was relishing his death or just stunned at what she helped do.
This would be null and void if it can be proven if the gem was on her necklace before old lady Tyrell came up to her (I don't remember). There's still some things that wouldn't make sense to us right now, but interesting nonetheless.

I didn't infer that he knew what was happening. There was a commotion, so he grabbed Sansa to protect her, because he holds her in such high regard now.

His quote of "If you want to life, we leave now." was too informative to just getting her out because of a commotion IMO.
He was RIGHT there within seconds of the coughing/choking starting. They didn't even know it was poison when he came to get Sansa.
Watch from 5:00 on, he was RIGHT there ASAP

So who ends up losing their head for this? Anybody or massive cover up. You would think somebody would have to even if they didn't do it.

I think it really depends, but I hope Tyrion isn't gone.
If it's Cersei or Tywin, I would think they would try and pin it on someone.
If it's not, I think they would also be out for blood.
So yeah, I guess I think someone gets killed for it.

I don't think they can afford a cover up. Tywin would think that makes him look weak. Vengeance/Justice must be served regardless of who is actually guilty. I think the most likely candidates are:
The Fool
Tyrion
Oberyn Martell
The Fool may not be a high value enough target to satisfy Cersei. It also might be embarrassing to the Lannister name to have a king poisoned by his fool.
Tywin hates Tyrion but also is very protective of him as he is a Lannister. While I can see GRRM killing off Tyrion in this way, I can't see Tywin letting it go down like this unless he sees this as a chance to cleanly disown Tyrion.
Oberyn Martell came to the wedding to kill Lannisters. Even if he's not guilty he would make an excellent fall guy and could possibly lead to more drama and a new House to have conflict with being that Cersei's daughter is in Dorne.

If I had to guess I would say Sansa but they don't immediately catch her on purpose sort of. I'm not sure about anything really. This one is a crap shoot.

Olenna has been pushing hard to get her to come to High Garden. But she'd basically have to go into hiding and assume a new identity until Arya kills all the Lannisters.

I believe the old lady Tyrell had something to do with it because in the previews for next weeks show shw appears to be telling margeary(sp) tha she is now the most powerful person in the kingdom. Am I the only person that noticed that?
