Having issues getting my old mini SD card into new phone

The memory on my S5 is full so I went to my old phone to grab the mini sd (32 gig). My old pics and stuff are not showing up on the new phone and deleting them is not an option...tons of nudes and shite i want to keep forever.
How do I put the mini sd in the new phone and keep everything the same?

Put the SD card in a PC and save what youy want.

It might help if it was a microSD card since that is what the phones use.
