Injuries to the 46-year-old referee were originally considered minor, according to the Deseret News, but doctors discovered severe internal head injuries when the man lost consciousness at a nearby hospital, where he reportedly remains in critical condition.

Generally, it's a really low level that's not that serious but still organized. So yes, it was a real referee
ETA: They seem to have chosen a nonspecific term. There's really no telling how organized the league is, but it is almost assuredly a step up from a pick-up game in organization.
