What do you want to see changed with new healthcare policies/laws?

Trump's "healthcare for everybody" comment got me thinking about what specific policies within the Affordable Care Act that I would like to see modified or done away with when new healthcare laws are developed.
1. Allow the sale of health insurance across state lines. Current laws have basically created monopolies in many states (like Alabama, where BlueCross BlueSheild is the only option in most areas). Doing this could allow competition, and would likely provide at least a modest decrease in prices.
2. The ACA requires that everyone who is on the same plan pay the same premium/rate, regardless of their medical condition. This means that people who have numerous health problems will pay the same rate as those with few health problems. It sounds harsh, but this policy needs to be destroyed. This is one of the main factors as to why premiums rise. Because everyone pays the same rate, it basically leads to a situation where the insurance company has to shell out more than it is allowed to bring in. Also, as premiums rise, subsidy amounts must also increase. Subsidies are paid for with tax dollars, particularly increases on the wealthy.
3. Don't set a minimum amount of insurance that can be sold. The ACA, as I understand it, sets a minimum amount of insurance that can be sold. Meaning it does not allow someone to have the option of buying only the things they need, if they choose to do so, and if what the need has less than what the minimum requires.
4. The most ambitious one yet. The government does not need to sponsor or mandate plans that are available to citizens. They government basically ends up limiting the number of plans that health insurance companies can provide. This led to many people losing their plans when the law was implemented, and to people have to buy plans that had more than what they needed. Let the insurance companies develop more plans, which would provide options that would better fit a larger number of people.

Government subsidized health savings accounts. End of life balances may be transferable to heirs. If the government is going to be deep in the healthcare business, we might as well incentivize saving, budgeting, and selective spending.
Children born from Medicaid mothers get HSAs set up for them at birth, with a portion of the savings coming from any of the government funded income they may be receiving.
