That would be up to the individual citizen in my system, and illegals would have no option for health coverage.

Not bad, I would add a government safety net for catastrophic illness, I've seen too many working class stiffs retire, get sick and lose everything they have worked for.

There would have to be a means test based on net worth before one would qualify for the government safety net. If you get sick and did not qualify due to net worth being to high, once your net worth is below that threshold due to medical costs you would then get government assistance.
