Growing up in the mid 70s, I watched a replay of a movie from 1957 called 'Curse of the Demon' with Dana Andrews.
It's about an American psychologist who goes to London to investigate claims of actions of an Occultist professor, and stumbles upon an ancient writ or parchment that summons demons. This writ or slip of ancient runic paper follows people and the sound effects of the 'demon' coming out of the air scared the bejeezus out of me...
If anyone would remake it now, with the special effects we have these days - it would be a blockbuster.

Michael Jacksons Thriller video.
Man, I remember when that shite debuted on TV. I watched it at a friend's house with his family and it scared the fricking shite out of me.
Also, I've told the story before on here, but I was a young kid (maybe like 4) and I saw this skit on some random sketch comedy show song called "The Gas Station Wash Room" (found out a while back it was from a Canadian group called The Frantics...they're more popularly known for their "Boot to the Head" sketch) and it terrified me. It was just some stupid comedy song (you can listen to the beginning of it here), but the video scared me half to death. I kept imagining like blood all over the floor and everything. Gave me nightmares for weeks.

It wasn't a movie but the Crypt Keeper was my childhood nemesis.

Event Horizon
Watched it by myself at night when I was 13. Still the only movie that freaked me out.

His dad had a "box" and got movie channels (and spice) for free.
I was freaking terrified. Really, really wanted to call my parents to get me.
