Kingslayer is obvious, but not really subtle. Anyone could have come up with that within seconds of Jaime's actions. I'd bet it was the first thought that went through Ned's mind when he saw Jaime sitting on top of the Iron Throne with the bloody sword.

Littlefinger I think was the most brilliant nickname that Martin came up with. A little finger is known as being the most harmless part of the body, and LF seemingly is harmless. He's a very small man with no titles and seemingly no men of loyalty to him. He even encourages this by adopting a harmless bird of the mockingbird as a sigil and doesn't seem to take his nickname personally as Tyrion does (even though Tyrion tries to shrug off being known as the Imp). I think when Martin was coming up with the books, he had the Baelish's hail from the Riverlands, he then came up with the fact that his nickname largely derived of his geography, being the smallest lord of a horrible fortress of the Vale.

Deep down though, Littlefinger is the single greatest threat to the realm (maybe next to the Others) and is a savage monster underneath his faux-affability. His true sigil is the Titan of Braavos or a savage giant. The geography on where he grew up dictates his heart as rocky and full of sheep's dung. I personally think he's the most evil person in the series, since unlike say Joffrey and Ramsay who are just insane, I think Littlefinger is perfectly sane and just wants to put everyone down to his level and destroy them all. That is why I think he has the best nickname of any character.
