No easy way to do it. You have to go online, print out the forms they require and go to the SSA or mail in the application. You get a temp one if you go to the SSA and they mail you the new card in the mail. Takes about 10 days from the time you print out and take it to the office.

They have the forms there - but you would still just get a temporary card. It has the same functionality though.
