I am rebuilding my garden at home for a variety of reasons. Where should I start looking for top soil? I have a 30ft by 40ft garden. How much should I get and where? I know what to do once I get it (soil samples, etc)

My recommendations is see if you can get your hands on a small load of mud, enough to fill a truck bed. I would place on top and smooth out. Add 1/3 of manure. I try to get my hands on 2 year old manure that had decomposted. I also like to sprinkle some grass trimmings that are about a month old. That will give it some nitrogen. Mix it all together and you should be good to go on a garden. You can get soil samples and place in packets from the LSU to get tested. Think its around $30 per sample. If you do that, I'd get 3 samples from different areas of the bed.

+1 on the manure. I usually get a few small trailer loads each spring from my Moms barn Spread it over the garden area Cover with leaf / grass clippings from the previous summer and then till it in. Second trailer full usually gets mixed in with the compost pile for adding as needed during planting.
