Outdoor TV Planning

I came across this photo on Facebook (A Plus Construction in LaPlace). Really nice set up. What would you plan in to something like this (in ceiling speakers, etc.)?

Trying to figure where to place it in the den as well because of this. I've noticed a lot of people center the fireplace and put the TV in a built in to the left or right. Problem with this is that the TV is no longer centered. I think I may try to put the inside fireplace in a corner.
