If you're not able to pay, the hospital charges you for the actual cost, rather than the inflated prices paid by insurance.

well also... a public hospital like UAB will still charge you an inflated cost, but if you don't pay any of it, that is all a part of their own "book keeping". That $200k probably paid for a few "non-paying customers" as well.
