If you have a medical condition, insurance covers the procedure like any other. Sometimes the foreskin can stick (I can't come up with the medical term right now, starts with a p) and the fix is a circumcision. That's covered. If you just want it and there is no medical reason for it, you'll be paying for it OOP most likely.

If you're serious:

For a kid- usually $150-800 cash, depending on who you go to and it's an in-office procedure

For an adult- you'll be put under and pay a lot more. And insurance probably won't cover it.

Write the insurance a strongly worded letter about how you like to sleep around with loose women, don't like condoms, and that a 65% reduction in HIV risk now is probably worth their money on HAART in the future.
