Yeah, it can be nasty. They have a really hard time healing anything in their extremities, which makes them susceptible to all kinds of stuff like infections and whatnot. The meds can help prevent it all, but only if they are taken correctly and the people make changes to their diets.
It runs in my family (white and not overweight), but everyone has avoided that extreme of a consequence by managing it with meds and behavior modification.
